Curious if this is happening with anyone else?
Before 9.0, the Mend Pet on my action bars would turn into Revive Pet if my pet died.
After 9.0, this no longer happens and I have to have a second action bar slot taken up for Revive Pet.
Anyone else experiencing this?

Strange, works on mine too. I removed all addons and macros, and this is what happened. Any addon/macro shenanigans?

It has worked for me, but there were occasions where it wouldn’t transform for 10 to 20 seconds
Re: Mend Pet Not Turning Into Revive Pet?
I have noticed this happened on my new characters made after 9.0. I noticed as I went through Exile's Reach that a few of the "Pet Utility" button (the one that makes a drop down in your spellbook for Mend Pet, Dismiss Pet, Tame Pet, etc) seemed to act like I had to put Mend Pet 2x on the bar or the pet wouldn't revive.
In order to fix it, once I finished Exile's Reach, I removed all the Mend Pet (and other pet utility aforementioned) and Call Pet off my bars. Then I dragged fresh copies out of my spellbook. That seemed to fix the issue for me, so Mend Pet changes properly soon as the pet dies to Revive.
On my main 50 Hunter, Mend Pet worked just fine to switch to Revive Pet. It only happened (personally) when I made a new Hunter after 9.0.
