Just a little update, it's been a few weeks of BIG breakthroughs for him. He's decided that he's a big boy now, and he goes potty by himself. I even taught him how to cover up after himself, and he's pro at that.
Just now, this old traumatized kitty was caught ...PLAYING! he was running and scampering and batting around a little crinkle ball I got him.
I'm so happy for him.

You're one of the most patient people alive, you realize this, right? Most people wouldn't tolerate a cat that needed so much help.
I'm so, so glad he's doing better. I had a cat at one point that had a lower back/hip problem, and I ended up having the vet (she's awesome) heal him up. Apparently someone else who went there for their cats fell in love with him, and took him in once he was better - we let her keep him, because he would be the only cat in her home and she adored him. I'm really glad you got your kitty happy.

My heart really goes out to you for rescuing this little guy...
I don't know if this will help or not, but here goes... after trying seemingly everything one of my cats still wasn't 100% housebroken, until my wife thought to replace his litterbox with a couple shallow storage tubs (like maybe 2' x 4' x 4" or thereabouts, directly side by side); since then he's been pretty good about going where he's supposed to - apparently he needed a larger / more accessible space to do his business (more space to spread out and also the rim was much lower so it was easier for him to get in and out).
