The program scans for the rares it has in a saved list. When it finds a rare it gives you a audio and pop up alert. It will only do so if your cache is cleared however. You only need to delete your creature cache and this can be found under World of Warcraft\Cache\WDB\enUS

If your on a Europe server then it would be World of Warcraft\Cache\WDB\enEU (I'm guessing on the Europe one)

What kind of beep is this going to make? I don't want to flip out if I hear a loud noise. Also so I did that all and I clicked on the rares I would like to be found- does it just beep when its spawns? Or do I have to go to the place first?

You should also download NPCscan overlay. It shows you on your maps where the spawn areas of the rares are. It's very handy!

You can change the sound it makes, but it's default is sort of a low, blaring horn. You have to be close to the mob for it alert you. It will not automatically tell you when something has spawned. Also, since it works off of your cache, once you have encountered the mob (whether in the wild or tamed in a city), you have to delete the Cache folder if you want to be alerted to that particular mob again. The edges of your screen will pulse red and it will say NPC FOUND with a little 3D model of who it is. If you click the model, it will target the mob for you so you can see if it's tagged or dead or free for the taking.

Hope this helps!

Personally, I would have described it slightly differently. Something more along the lines of "a thunderously loud attention-grabbing horn which is only surpassed by the thumping of your heart as you frantically try to remember how to land, and where you put the Tame Pet button".

Or maybe that's just me.

I'd describe it as a "loud, low horn that makes you go "Where's my Pyroblast..."".

I forgot to mention it will make your screen flash red. Here is a You tube clip of it's sound.

Here is a review of the add on as well and how it works:
