My original EQ Paladin was Calidain Stormheart! I would love for WOW to insert surnames into the game so I could continue the Stormheart legacy with Caanin Stormheart and his dwarf cousin Deniro Stormheart!!!
Sadly I doubt we will ever see it happen in game!!! I'd love more cosmetic modifications in game as well, such as hoods on cloaks (most helmet models are hideous, I turn them off) and how about the options to dye our armor, why does everyone have to look exactly the same??? Honestly, if I could dye my armor, my Worgen would always dress in dark, gothic like colors!!! As for the hoods, I created a Draenai Death Knight and never leveled him (never caught on with me) but I never deleated him simply because he looks so damn cool with his dark armor and hooded look!!!

I'd love for my RP gnome to be able to display her last name (Nira Bottlerocket), but I'd drive myself crazy trying to come up with last names for some of my non-RP characters I've never had to think about. My main, for example—what the heck do draenei last names sound like? Epadoa isn't that draeneish a name anyway but would I continue with her theme and make her last name S'Armunai (would apostraphes even be acceptable for surnames, since they're not for given names? Sarmunai just looks dumb)? Go for a different fandom and do something like Epadoa Waterstrider? Use my de facto name and go Epadoa Dys'daera? None of those sound right—Epadoa doesn't HAVE a last name in my head, but I'd feel stupid not having one defined if the option was there.
Well, most draenei don't seem to actually have any last names, so you'd probably do just fine if you didn't pic one.
I'd have some trouble with the majority of my chars too, though: They don't typically have great RPish names. They're not stupid names like Killusoon or something like that, but names like Saturo, Decay and Stereo don't exactly lend themselves to surname RP. :/
Surnames would be a mixed blessing as has already been mentioned. I for one wouldn't use the feature but I know many would.... and many would abuse it.
Let us not forget, WoW's very own "Tigole" was originally EQ's "Tigole Bitties".
I played Everquest 1 and miss having surnames too. One of the funnest things in that game for me was hitting Level 20 so that I could think up a good surname for that toon. Among my favorites:
Sigrah Silvermane - Vah Shir Beastlord and by far my favorite. Was also known as Sigrah the Beast.
Aneron Rathskeller - Human Cleric. He was my Cooking & Brewing character.
Aebric O’Dell - Human Wizard. He was my “Irish” toon.
Swampfever Pete - Troll Beastlord. Cleverly used the surname command to add “Pete”).
Blackmarket Goods - Half Elf Rogue. He pretty much served as my “Seller” toon in the Bazaar).
As for the whole "people would abuse it" thing, that's not a good enough reason to *not* implement surnames. I say that because people will find ways to abuse anything. Just last night, I saw someone in trade paying people to sign his guild charter so he could have a guild named "I Beat My Wife". There's also no shortage of people who go out of their way to name themselves something crude or otherwise offensive. But honestly, I could care less on non-RP servers. If it bothers me, I put them on ignore and avoid them. Besides, for the most part, I'm only going to meet 99% of them in a city and spend all of 10 seconds seeing it.
You know, I was thinking, if Blizzard is afraid the feature would be abused - either by people who opt for vulgar names, or if they take issue with ones wanting to be named Wrynn, Hellscream, etc., maybe it could be a feature that is only unlocked after hitting a high level - say, 50, 60, 70, or maybe even all the way up to 85. That way people couldn't just start out with a vulgar name. It wouldn't stop it all, but it might deter it some.
As for me, I do have surnames for my characters: Constantyne's family is Dawntreader, and Whitnee's maiden name was Kingsley.

I fail to see how a surname feature could be any more abused than a character's given name, pet names, or guild name. There are already filters in place in the game; you can't use the same name as a lore character (my guess is that only certain surnames, such as Wrynn, would be blocked), and the same is probably true for obscenities.
That said, I would probably be finding myself in a situation similar to Xella; I don't have surnames thought up, but it would bug me too much to pass up the opportunity.

hehe i remember just rushing to lvl 20 in everquest to get a surname , that and housing is the thing i miss the most.. but tbh i miss surnames more then my eq2 house, cause i didt use the house to much.. just designed it and i loved seing pictures of others houses and the genial ideas they come up with... but if i HAD to choose i would choose surnames ... it just feel like my char is missing something.
