I was using my lower level druid to collect Noblegarden eggs in Kharanos, and I was running behind this one building and something possessed my to climb on top, and I fell into a furnace/chimney thing. I was jumping all around the edge desperately trying to escape, I tried in travel and cat form but I just couldn't get out. I was so embarrassed and hoping no one else saw, but then I remember I could use the stuck thing and I was soo relieved. (And no, it never did once occur to me to use my Hearthstone, and I thought my character would be stuck in that furnace forever. lol.)
I get really hyper when I get stuck. xD

God, I've had so many days of fail as well. Clicking wrong targets, sending my pet after wrong targets, dropping an explosive trap when I meant to drop a freeze trap... that one's always fun when you're trying not to aggro the mobs and just want to trap them (I only did it once!).

Oh hey, that reminds me of another one!
My first time in heroic Deadmines. We're going through the dream sequence and I'm told "OK, after this fight we'll be surrounded by fog and attacked by spiders. DON'T MOVE because if you do you'll aggro more spiders and we'll all die."
So what do I do? Drop an explosive trap...which aggros a whole bunch of spiders. We didn't all die, but most of us did.

Runner up: tamed the Strider Clutchmother back in vanilla and managed to convince (con) my friends into doing several instances with me to level her up. Hey, I was 60, and she started at 20! That was a LOT of levels to have to solo by onesself! I proved it could work just fine in Lower Blackrock Spire, so afterwards we trundled off to Scholomance. Cleared most of the dungeon no problem, making our way downstairs, finally reaching Alexi Barov. While setting up, I happened to inch closer so I could stare at him to orient myself and prepare.
Only I inched up into his aura range. His pulsing shadow damage aura's range. With my pet on Defensive. No, I didn't have time to hit the "oh god! Abort! ABORT!!" button before Chickayn had bravely charged Alexi to defend my honor. The warrior tank of the run absolutely never forgave poor Chickayn for that wipe.
The winner, if only measured by the amount of personal humiliation involved: tried out for a raiding guild during Wrath of the Lich King, when Naxxramas was the raid you could bring applicants and trial members to to get a good sense of how they stacked up. I wouldn't say I have the loftiest standards to feel I'm doing well, but I seek to avoid the stuff that would lead to a raid leader slapping their forehead and groaning "how could they DO that?!" Bring flasks, have some food, don't butt/pet-pull an entire room by accident. Pretty standard things if you're trying to raid. Easy! For most of the run, I pulled all of that off just fine. I was nervous, but felt I was making an adequate showing. The hunter officer even seemed pretty happy, comparing me favorably to one of the member hunters in the run.
So of course when we reach one of the remotest, "this is not a good time to have stuff come up" bosses, for the first time in the history of me playing the game... my bow breaks at the start of the boss fight. I don't know what's most impressive: that I didn't die of shame, or that they recruited me anyways.

Ah, my shaman had a run in with the Elevator Boss in BWD tonight...sister distracted me on ICQ asking how the raid was going, the rest is history...
Hunter fails, where to even start....disengaged to death in SP. fairly tame for here I'm seeing...more than my fair share of missed Misd or Trap Launcher, just plain misjudged trapshots due to poor depth perception, shoot past/cast past bugs as a mob dies...
Outran my pet trying to finish Arch shaman boss, does that count?
